School Wellness Program
The School Wellness Program at Perry Middle and High Schools is an initiative designed to support the health and well-being of our students by providing them with access to professional health services right here on campus.
Available Services
The program includes access to both a nurse and a therapist, allowing students to receive medical and mental health support during school hours. This ensures that students have convenient access to the care they need, helping them to stay healthy and focused on their education.
Consent Form Requirement
To take advantage of these services, it is essential for each student to have a signed consent form on file. This ensures that we have parental or guardian approval for their child to receive services from the nurse or therapist.
How to Access the Consent Form
Online Link: If you did not receive the postcard, you can fill out the form directly through this link: https://form.jotform.com/230604200541034.
In-Person Copies: Physical copies of the consent form are also available at the main office.
Importance of the School Wellness Program
This program is an important addition to our schools, providing essential health services that can enhance students' overall well-being and academic performance. We encourage all parents and guardians to complete the consent form to ensure their child can benefit from these services.
For any questions or additional information about the School Wellness Program, please feel free to contact the school administration. We look forward to supporting the health and success of our students through this program!
